Community Resources
Community Resource Program
The purpose of this program is to assist people to find health care services and programs they are eligible for.
The goals of the Community Resource Program are:
- To assist clients in navigating the Medicare Healthcare system
- To assist with enrollment in Medicare D (prescription coverage)
- Acts as a mediator with:
- Medicare companies
- Social Security
- Prescription Assistance Programs
- To assist lower-income clients with extra help and/or access to medications
- To assist individuals through the Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) process
- Refers to other programs/agencies as needed
