Community Resource Program

This program focuses on assisting clients with the complex Medicare system and also assists clients in finding programs that may assist with various health care expenses including expensive medications.

The goals of the Community Resource Program are:

• To assist clients in navigating the Medicare Healthcare system
• To assist with enrollment in Medicare D (prescription coverage)
• Acts as a mediator with:
  • Medicare companies
  • Social Security
  • Prescription Assistance Programs
• To assist lower-income clients with extra help and/or access to medications
• To assist individuals through the Social Security Disability Insurance (SSDI) or Supplemental Security Income (SSI) process
• Refers to other programs/agencies as needed
